| After final submission of the form, the applicant must save the pdf file and take a print out of the same for future reference. The form must be submitted online only on the specified dates alongwith the enclosures mentioned. | |

| IN CASE OF SELECTED CANDIDATES ONLY, THE HARD COPY OF THE FORM ALONGWITH COPY OF ENCLOSURES MUST BE PRODUCED. THE ORIGINAL DOCUMENTS MUST BE PRODUCED FOR VERIFICATION AT THE TIME OF ADMISSION. IN CASE OF ANY DISCREPANCY, THE ADMISSION STANDS CANCELLED. |
